Upon completion of your examination and consultation with Dr. Casciato , you will be provided with an estimate of your proposed treatment.

Dental Plans

Dental plans in the marketplace are too numerous and varied to allow us to know the details of all of them. Your particular plan may or may not cover the full costs of your surgical treatment as many insurance plans only cover the Dental Association General Fee Guide, and not the ODA Fee Guide for Certified Oral and Maxillofacial Surgeons. As well, there may be certain procedures that are not covered through your dental plan. These factors are beyond our control.

As a service to you, our office will electronically submit a pre-determination to your insurance company on your behalf in order for you to better understand what your insurance plan will cover.

Payment Policies

  1. We offer flexibility of payment by cash, Visa, Mastercard or debit card. Cheques are not accepted.
  2. On the day of surgery, if you have received written pre-authorization from your insurance company, a copy of this must be brought to your appointment in order to directly bill your insurance company. You are responsible for payment of the balance not covered by your plan.
  3. If written predetermination has not arrived prior to the surgery date, or if you are not covered by dental insurance, we request full payment on the day of surgery.
  4. If your dental plan will only authorize payment to you, then we request full payment on the day of surgery.

If we do not receive payment from your insurance company within 30 days following surgery, you will be billed directly for the full/remaining fee.
